This waiver applies to all events/practices/gym/tournaments/matches held at the Infinity Sports Zone Inc. Facility located at Unit-7, 1160 Birchmount Rd Scarborough, ON M1P 2B7 (hereinafter called ‘Infinity Sports Zone Inc.’), at any branch of Infinity Sports Zone Inc. or organized by Infinity Sports Zone Inc. but held at any other location.
In consideration of being allowed to participate – I, the undersigned, acting on behalf of myself, and any participants under my booking (hereinafter collectively referred to as “Participants”), hereby agree:

• To ensure that prior to participating, all Participants will inspect the facilities and equipment. If I or any Participant believe anything to be unsafe, I/we will immediately advise the management of Infinity Sports Zone Inc.
• Understand and acknowledge that participation in activities at Infinity Sports Zone Inc. involves inherent risks, including but not limited to, the risk of injury and/or death. I am aware that sports activities can be physically demanding and may involve sudden and unexpected movements, collisions, and other risks not present in everyday life.
• Acknowledge and fully understand that all Participants are voluntarily participating in activities that involve known and unknown risks of injury, including catastrophic injury, permanent disability, and/or death, which might result not only from the actions, inactions, or negligence of the Participants, but also from the actions, inactions, or negligence of others, including but not limited to Infinity Sports Zone Inc., the rules of play, the conditions of the premises, or any of the equipment used.
• In consideration of being allowed to participate in these activities, I, on behalf of myself, my team, and all Participants, for our heirs, executors, administrators, and assigns, do hereby release, discharge, and hold harmless Infinity Sports Zone Inc. facility, its officers, directors, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action, whether presently known or unknown, arising out of participation in these activities.
• Assume all the foregoing risks as a condition of participation and accept personal responsibility, on behalf of all Participants, for the damages following any such injury.
• Unconditionally release, waive, indemnify, and hold harmless and consent not to sue the officers, directors, administrators, agents, coaches, other employees, volunteers, sponsoring agencies, sponsors, and advertisers of Infinity Sports Zone Inc., on behalf of all Participants.
• Accept responsibility for all medical expenses incurred by any Participant, whether or not covered by insurance. In case of emergency, accident, or illness, authorize ambulance transport to the hospital. Authorize physicians, athletic trainers, technicians, first-aid personnel, nurses, and dentists to perform any diagnostic, treatment, or operative procedures and x-rays. No guarantee has been given as to the results of examination or treatment. I, on behalf of all Participants, accept total responsibility for any and all medical costs.
• Accept responsibility for the decision to allow any Participant to continue participation if suffering from injuries.
• Acknowledge that Infinity Sports Zone Inc. is not responsible for any personal items or property that are lost, damaged, or stolen at the facility.
• I understand that all Participants are responsible for any damage to the facility or property caused by willful misconduct, negligence, or intentional acts. I agree to cover any costs associated with repairs or replacement.
• I have read and voluntarily signed this liability waiver and release of claims on behalf of all Participants and further agree that no oral representations, statements, or inducement apart from the foregoing written agreement have been made.
• I hereby release, waive, discharge, and covenant not to sue Infinity Sports Zone Inc., its owners, officers, employees, agents, and representatives (hereinafter collectively referred to as the “Released Parties”) from any and all liability, claims, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury to vehicles or their contents while parked at Infinity Sports Zone Inc., whether caused by the negligence of the Released Parties or otherwise. I agree to indemnify and hold the Released Parties harmless from any and all liabilities, costs, expenses, claims, and demands, including legal fees, arising out of or related to the use of the parking facilities by any Participant at the facility.
• I understand that all Participants are permitted to park their vehicles only in the designated parking area specified by Infinity Sports Zone Inc. and agree to comply with all posted signs and instructions regarding parking regulations.
• I read this Liability Waiver, fully understand its terms and conditions, and acknowledge that I am giving up substantial rights, including the right to sue the Released Parties. I acknowledge that I am signing this waiver freely and voluntarily on behalf of all Participants and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
